My theory is that those weird audio jumps, where the sound suddenly goes out of sync and is from earlier in the scene, is used to cover up on set dialogue by the director, etc. I guess they don't want a voice saying "OK, now flip her over and go to piledriver" or whatever - although personally I find the audio jumps just as distracting.

You actually gave him the correct answer to that. These scenes are directed by someone behind the camera and it happens that he might speak to communicate what he wants to the performers, change of positions and even male rotation and remember we usually shoot without interuption (one take) by the exception of pee scenes.
