I think at least some of the reason they are rare is that while they may be exciting as an idea to fantasize about, in practice they don't shoot well (and what doesn't shoot well doesn't sell).
Firstly, performers who want to be crippled by attempting this are rare. And very wise they are too.
Second, in practice nothing really fits in (not supposed to really). So you get maybe two studs half in and one frustrated hopeful 'chipping away' at the edge (it looks crap).
And thirdly, you need to bear in mind that for each 'tool' involved, there is a fully-grown man attached to the other end.
In the studio this means directors are presented with a wall of sweaty human backs and rubbing asses and very few options as to how to get lighting and cameras on the point of interest.
TAP is a classic example of more is less. Fantasizing about it (if that is your thing) will be a million times better than you will ever see it.
