That's a problem for the camera man to handle. The director needs to instruct the actors how to move on set so they don't obstruct the view of the camera man, what to do when it's not their turn etc. So teamwork is necessary to ensure that what ends up being captured is a pretty girl getting airtight all the way by random big "dudes".
