Most of what is referred to in porn as prolapse is not true medical anal/rectal prolapse and is not an issue at all because it's something that is controlled. Actual prolapse is uncontrolled and happens even when not deliberately loosened up and gaping. Anal sex, even extreme anal when practiced safely, has no link to causing true anal prolapse. It seems to be genetic and tends to happen to certain people (quite rarely overall) regardless of what they do. In those few unlucky people, it's more likely to happen when straining on the toilet than when having sex. But the actual act that first triggers it usually isn't even the actual cause, it was going to happen anyway.
So most porn girls need not worry even if they do extreme play and develop the muscle control to loosen up their anal muscles enough that they can push out a little bit of a rosebud. That's not at all the same thing as a real prolapse.
